My School, My Voice Survey

Parents and Guardians-

We are writing to let you know about a survey your child will be asked to participate in
this winter, called My Voice, My School. This survey is conducted by the Chicago Public Schools and the University of Chicago Consortium on Chicago School Research (CCSR). The survey asks students about their experiences and feelings, such as support from their teachers, how safe they feel at school, their school work, and plans for the future. Having such information directly from students is critical for developing plans to improve schools.

For information about this survey, please visit: http://www.cps.edu or call the survey hotline at 1-866-440-1874.

All students’ answers will be kept in strict confidence. No students will be identified in reports relating to this survey.

Please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c)(1)(A), you have the right to review a copy of the questions asked of your student(s). If you would like to do so, you can view a copy of the survey at your school.
