Proof of immunizations and exams are due at school by October 15, 2015. If you have questions about what your child needs for school, please call your school's main office. Schools will help identify what your child needs to be in medical compliance. Also, ALL students must turn in the Student Medical Information Form to their schools at the beginning of the year.
